Health Requirements

All 51ÂÜÀò students (excluding those in online-only programs and Feinberg) are required to submit the following health forms/vaccines upon admission:

  1. Health History Form
  2. Meningitis Advisory
  3. TB Screening Questionnaire
  4. Notice of Privacy Practices
  5. Upload a copy of Immunization Records

*Required: 2 doses of the MMR vaccine (Measles/Mumps/Rubella) given at recommended intervals for all in-person students, including graduate students.

*Required: The Meningitis ACYW135 vaccine given on or after the age of 16 is also required for all students living in 51ÂÜÀò-owned housing (with the exception of married housing.)

    All traditional undergraduate students, undergraduate and graduate students on an F1 visa, undergraduate and graduate students residing in 51ÂÜÀò-owned housing, all students at the Rosemead School of Psychology (undergraduate, graduate, and doctoral), and Master of Arts in Marriage and Family Therapy (MFT) students within the Talbot School of Theology (regardless of unit load) are required to either purchase the university-provided Student Health Insurance Plan or waive this requirement by providing proof of comparable health insurance coverage. Eligible students will be automatically enrolled in the Student Health Insurance Plan, and the cost of this policy is subject to change.
